POP WARNER FOOTBALL & CHEER

REGISTRATION INFORMATION

Registration MUST be fully completed prior to start of season

Practice starts August 1st 2024 6-8:30 pm. Practice will be Monday - Thursday (younger teams may practice less).  Once school starts practice will be 3 days per week. Games are usually Sunday mornings. Players must bring their own water bottles. See links below for paperwork needed. 

You now can upload your child's birth certificate, the Pop Warner Forms, and report card during online registration. Please note that the Physical form has changed, Pop Warner is no longer requiring a physical to be done. You will need to fill out the  2024 Pop Warner Physical Fitness Medical History Form certifying that your child has had a recent physical and they are medically able to participate in the program.

Paperwork Required for Registration:

1. Online Registration completed and Paid by CC - If not paid, Cash or Check Due Immediately

2. 2024 Pop Warner Physical Fitness Medical History Form completed by parent - MUST be current year

3. 2024 Pop Warner Player Contract completed and signed by Parent 

4. 1.5"x1.5" Photo - Unless a new photo was uploaded online during registration (yearly)

5. Original Birth Certificate w/Raised Seal - This will be held and returned upon certification by CJPW

6. Year-End Report Card from previous year - 2 copies needed

7. $100 Work Bond payment (one per family) - CASH ONLY - Each family must work a minimum of 4 hours either at the field or at the Youth Sports Festival. Duties may include, but are not limited to the following: Snack Shack (Must Be 18), Game-Day Chains, Coaching, Team Parent, Fundraising, etc. Once your hours are logged and verified, your Work Bond payment will be returned with a receipt. Payment will not be due if the hours are worked at the YSF since this is prior to the start of our season. Failure to fulfill this obligation will result in forfeiture of your Work Bond Fee.

8. Equipment/Uniform Security Deposit Check made Payable to: Adams AC, post-dated to October 15th. ONE CHECK PER CHILD. This check will NOT be deposited unless your child's equipment is not returned, or it is returned damaged at the end of the season. Fees and expectations are as follows: Cheer - $200 - Cheer uniform (Shell, Skirt, Bodyliner & Jacket (if received)) are due back at the end of the season dry cleaned, on a hanger, in the plastic. Flag - $100 - Flag Jersey and Pants are to be returned clean and unaltered at the end of the season. Football - $250 - Practice & Game Jerseys & Pants are to be returned clean & unaltered at the end of the season, along with assigned Helmet (including chin strap provided) & Shoulder pads (including rib pads or back plate if provided) Failure to return the equipment or failure to return it as listed will result in the check being deposited.
